MyMidMichigan TERMS AND CONDITIONS OF USE MidMichigan Health is pleased to offer our patients online computer access to portions of their medical record through the MyMidMichigan web portal. If you would like access to this service, you must complete an online request and agree to these terms and conditions. MyMidMichigan is not for Emergencies. MyMidMichigan allows you to access your medical records and to communicate with MidMichigan Health about non-urgent matters. You should NEVER use MyMidMichigan for urgent health issues. If you are having a medical emergency, go to the nearest emergency room or dial 9-1-1. MyMidMichigan User ID and Password Accessing and using MyMidMichigan is voluntary. If you choose to use MyMidMichigan, MidMichigan Health will assign you an activation code. You will need to create a unique User ID and a Password. It is your responsibility to protect your User ID and Password. Anyone with access to your User ID and Password may access your personal health information. It is your responsibility to change your password if you believe it has been compromised or shared with someone else. You should change your password at least every 180 days. Each time you log into MyMidMichigan, the system will retain an audit trail of your access. Secure Messaging If you send a request or message through MyMidMichigan, you can expect to receive a response within two business days. If your provider is not available when you send your message, we may route your message to a different provider. If you do not receive a response to your request within two business days, please contact your provider’s office by telephone. MidMichigan Health providers or staff may send messages to you through MyMidMichigan. The system may send messages to your personal e-mail account to tell you that you have a MyMidMichigan message. E-mail messages will not include personal health information. However, anyone who has access to your personal e-mail account may see the e-mail advising you that you have messages on MyMidMichigan. You may not use MyMidMichigan to request information or advice about someone else’s medical condition. MyMidMichigan messages will become a permanent part of the patient's medical record. Therefor, it is important that all messages sent or received from a patient's MyMidMichigan account are only about the patient. Confidentiality Federal and State law govern the confidentiality of medical information. MidMichigan Health is dedicated to keeping your medical information confidential. When you access your information, you must share the responsibility for keeping the information confidential. Please see MidMichigan Health Notice of Privacy Practices MidMichigan Health Right to Terminate Access to MyMidMichigan If MidMichigan Health discovers that you have shared your MyMidMichigan access information or that you have misused your MyMidMichigan privileges, we may terminate your participation in MyMidMichigan. MidMichigan Health shall not be liable for damages arising from a breach of confidentiality resulting from your sharing or losing your MyMidMichigan access information. MyMidMichigan Availability There may be times when the MyMidMichigan system is not available to you. For example, system maintenance, back-ups or failures may limit MyMidMichigan availability. We provide access on an “as available” basis and you are not guaranteed access at any particular time. MyMidMichigan Security Encryption: We use 128 bit Secure Socket layer to encrypt the communication from your web browser to our secure web server. Firewall: We store personal health information only in the secure electronic medical record, protected from the Internet by a firewall. Dedicated web server for MyMidMichigan: We separate the web server and electronic medical record machines, and we do not save any personal information on the web server. This protects your information in the unlikely event of an attack on the web server. No caching on the client side: We disable caching of data in the browser and force the web pages to expire immediately. This prevents a follow-on user from viewing pages that you had previously viewed while using MyMidMichigan. Session timeout: After a period of inactivity, the logon session will time out. Monitoring: We constantly monitor the web server for evidence of unauthorized break-in attempts. We apply the latest security patches, as they become available. Third-Party Links and Services You may choose to use third-party services, such as translation or location services, with your MyMidMichigan account. For your convenience, MyMidMichigan may also provide links or references to other sites. MidMichigan Health is not responsible for the content of these sites or services and shall not be liable for any damages or injury arising from using them. You may need to submit your information to a third party in order to use these sites. This could result in that third party using or storing a copy of your information. If you decide to use these services, you do so at your own risk and subject to their terms and conditions.
